New Patients
Thank you for choosing Atlanta Children's Clinical Center as your child's medical home, where we always provide a comfortable, caring environment so our patients and their families can feel at ease. We've included helpful items on this page to ensure you and your child's first trip to our office is productive, stress-free and enjoyable. If you are considering becoming a new patient and would like to meet your new doctor, we offer one complimentary "Meet and Greet" visit either in person at the office or via telemedicine, if you prefer. Just call the office or email us to schedule.

In order to provide the best possible care for you and your child, we will need all of your child's previous medical records. If you have a newborn, there will be paperwork from the hospital that you must bring to the first appointment about 2-3- days after leaving the hospital. If you have an older child and are transferring into the practice, we will need copies of your child's medical records from their previous pediatrician. This will likely include growth charts, immunizations, and office notes. We need these PRIOR to your appointment so we can be best prepared for your visit. You can request records from your previous pediatrician using the Release of Medical information to ACCC link below. Then you or they can fax the records (404-261-2669) or email them to us at [email protected] or drop them off in person. We will also need a copy of your insurance card prior to the first visit.
