Referrals and Authorization
Your insurance may require prior-authorization for medications or for and appointment or procedure with a specialist. We ask that you review your insurance policy and its benefits prior to submitting the request.
For appointments or procedures with a specialist, your child may need to be seen by one of our physicians first. Please verify with the practice of the specialist you wish to visit that they accept your insurance plan and then provide our office with the necessary information to complete your insurance referral at least 3 days before your appointment. Referral forms can be found under the Patient Forms tab.
For medications that require prior-authorization, your pharmacist will need to fax over the request form with complete information regarding the medication in question to (404) 261-2669. Depending on the insurance company, it may take up to 3 days to process this request.
